Minnesota's HMIS

HMIS Supports a Vision for Data and Performance Success Minnesota’s Homeless Management Information System (HMIS) is a web-based database used by homeless service organizations across the state to collect client-level data on households experiencing or at risk of homelessness. The administration of an HMIS is a mandate from the federal Department of Housing and Urban Development (HUD). HUD’s Office of Special Needs Assistance Programs outlined a set of goals for the field of homelessness assistance, and three strategy areas where communities should examine their current state and track progress toward an advanced state. HMIS plays an important role in communities’ achievement of these goals. • Strategy 1: Improve the capacity of people setting up, operating, and benefitting from data systems. • Strategy 2: Data systems collect accurate, comprehensive, and timely data. • Strategy 3: Continuums and partners use data to improve efforts to end homelessness.

NC HMIS Training and Resources The Michigan Coalition Against Homelessness (MCAH) is the lead agency for NC HMIS and provides training and technical assistance for all CoCs participating in the NC HMIS implementation. HMIS Learning Center Home Getting Started The HMIS Learning Center has been created to assist those looking for guidance and best practices to elevate systems of care to their fullest potential.

Sage HMIS Reporting Repository

Sage HMIS Reporting Repository User Manual The Sage User Manual contains information on how to use the Sage HMIS Reporting Repository (Sage) for HUD reporting. This includes how to create and manage a user account, how to navigate the dashboards, and how to use different functions and tools in Sage. This manual does not provide instructions for submitting specific reports like the APR or CAPER. • Note: Guidance for submitting specific reports can be found on their respective pages below. Annual Performance Reports (APRs) In April 2017, HUD began requiring CoC homeless assistance grant recipients to submit their APR data through a new online system – the Sage HMIS Reporting Repository. Grantees will no longer submit APRs in e-snaps. This change in systems applies to all CoC homeless assistance grants regardless of when they were initially funded. • YHDP APR and QPR Guidance Youth Homelessness Demonstration Program (YHDP) projects funded in round four and forward must provide quarterly reports. This document provides guidance on completing YHDP APRs and Quarterly Performance Reports (QPRs). • ESG Consolidated Annual Performance and Evaluation Reports (CAPERs) ESG recipients may submit their accomplishment data in Sage. This means that recipients will no longer need to use ESG-CAPER Annual Reporting Tool (eCart). Instead, project reports (Comma Separated Value (CSV) downloads) generated by HMIS or comparable databases can be uploaded directly into Sage. • Emergency Solutions ...
